Job Description:
We are seeking an energetic, patient, thoughtful, and outgoing Community Facilitator to support & assist an individual in the surrounding community. You will teach all the community. You will teach all community safety & awareness, social interactions & self-help skills.
Responsibilities:
- Provide one-to-one support to an individual in their home & community who has a diagnosis of Autism or other developmental disabilities & has limited safety awareness.
- Implement the individual's Support Plan as developed.
- Provide the necessary accommodations to support individuals with various challenges (behavioral, physical, communication, etc) within their home and community.
- Support the individual to establish a daily/weekly schedule of activities.
- Act as a mentor and role model for socially acceptable and culturally valued ways to interact and support each other to be successful.
- Participate hands-on in all activities.
- Assist and support the person to maintain their safety and well-being.
- Provide information to family members about the individual's unique needs to assist in building a more cohesive family unit.
- Comply with all policies and procedures of the organization and public locations of activities.
